Braden Scale Activity #2 Mrs. K is an 80 year old female in an long term care facility. Mrs. K has been experiencing fever with a current temp of 101.4. She is sweating through her hospital gown. Mrs. K is usually continent of bowel and bladder, however, she is having increased frequency of urination which is causing her to be incontinent at times. She is able to ambulate with assistance and a standard walker. Mrs. K ambulates 4-6 times a day, to and from all meals and to and from the bathroom. While in bed or a chair, Mrs. K is able to reposition herself independently. Her body sensations are intact. Mrs. K has a poor appetite, eating between 5-10% of her meals for the past 3-4 days. She drinks 300-500 mL of fluid daily. Upon assessment, her skin is intact.
